- Stonebridge Life Care & Wellness Services provide customized home and community care emphasizing the importance of maintaining and enhancing quality of life throughout the aging process. Trained and certified staff assist clients and families to collaboratively develop an individualized care plan and provide service with a keen attention to physical, emotional, spiritual and psychosocial needs of client and their families.All services and programs are supervised by Registered Nurses and a Registered Kinesiologist.
